Eligibility
Who Can Receive Our Services
We provide DD Supportive In-Home Services to individuals ages 5 and above with a primary diagnosis of a developmental disability who need extra assistance to live safely and independently at home.
Eligibility Requirements
To qualify, individuals must:
- Have a documented developmental disability diagnosis
- Show limitations in two or more adaptive skill areas (e.g., communication, personal care, mobility, behavior, or learning)
- Require support that exceeds what a family or unpaid caregiver can provide
- Be eligible for DD Waiver funding (Tier 1–4)

Exclusion Criteria
We are unable to provide services to individuals who:
- Require 24-hour nursing or medical care
- Have untreated substance abuse issues
- Exhibit behaviors posing immediate safety risks
- Have active contagious diseases or conditions requiring medical supervision
